2012 Application to Begin Lay Speaking
This form is used by a person who wishes to begin the process of becoming a local church or certified lay speaker. The application must include a recommendation by the pastor and the church council or charge conference.

2012 Partial Year Salary Reduction
Pastors who are changing appointments during the calendar year or who start a new appointment after Jan. 1 need to complete this form. The date of approval is the earliest date in which qualifying expenses may be included. Remember to pro-rate the amount of salary and reductions for the period indicated (i.e. half of the salary and reductions for a new appointment that is effective July 1).
2012 Salary Reduction Agreement for IGRC-sponsored Plans
This form enables a persons to have pre-tax dollars directed for payment of individual and spousal contributions for health insurance as well as setting up pre-tax dollars for flexible spending on healthcare costs or dependent care. Please refer to the instruction sheet to assist in completing the form.
