Form to be used and updated annually by all assigned CLMs as the basic covenant document between the CLM and their Mutual Ministry Team. The document must be signed by the Church Council Chairperson and Lay Leader of the congregation where the CLM is assigned, the CLM, members of the Mutual Ministry Team and approved by the District Superintendent and Equipping Clergyperson.
